Have you filled out the Participant’s Medical Information Form yet?
Gabriola Recreation Society needs to have a copy of the participant’s medical information on file for every participant attending an event. You must have either submitted our online Medical Information Form (link) or printable Medical Information Form (Adult Form – link, Child/Teen Form – link) more recently than September 1, 2017.
Once we have your participant’s medical information on file you won’t ever have to enter the information again. You can just check the box stating we have the participant’s information on file.
